CLAIM SUBMISSION
Filing a claim can be made by sending a 3 (three) documents, namely:
1. Medical letter stating the insurance of participants diagnosed with dengue fever.
2. Results of laboratory examination.
3. Proof of identity such as ID cards or a family card.

HOW TO ACTIVATE DENGUE FEVER INSURANCE

You can apply by yourself. Just activate PIN number which is printed on insurance member card, and send it by SMS to 9779 (Telkomsel, Indosat, XL) or 0877 588 70000 (other providers) with format:

ID card number (without dot or space) # Name of insured # PIN number # address of insured

ACA will send a SMS CONFIRMATION in short regarding the insurance periode, i.e 15th days after the date of activation. The insurance period will expire the next 12 months or once you receive the compensation.

HOW TO CLAIM YOUR DENGUE FEVER INSURANCE

If you suffered from the dengue fever, someone can report the incident by sending SMS CLAIM to 9779 (Telkomsel, Indosat, XL) or to 0877 588 70000 with below format:

ACA (space) PIN Number CLAIM # (no dots, no spaces) # Name of insure

Our claim officer will contact you as soon as possible to guide you for preceding the claim.
